Inchaban Nkwanta Community Health Initiative

Challenge

Before this intervention, many households in Takoradi faced significant barriers to accessing preventive healthcare due to financial constraints and limited access to primary care services. This resulted in delayed health seeking behavior, with many individuals only presenting when conditions had progressed, increasing the risk of complications from preventable diseases such as hypertension and diabetes. The core gap was limited access to affordable preventive healthcare and inadequate health insurance coverage within underserved communities.

My Role

Designed a community centered preventive healthcare intervention to improve access to screenings, early detection, and health insurance coverage.
Led stakeholder engagement with community members and health personnel to align implementation with local health needs.
Coordinated partnerships with healthcare professionals and volunteers to mobilize medical supplies and logistical support.
Early detection of conditions such as hypertension and diabetes improved opportunities for timely intervention and reduced long term complications.

Impact

More than 300 individuals received preventive health screenings, laboratory services, and medical consultations, improving early detection and timely access to care.
Over 250 community members were enrolled in or renewed their National Health Insurance coverage, strengthening financial protection and continuity of care.
More than 700 kg of clothing was distributed to vulnerable households, addressing social determinants that influence health outcomes.
Early detection of conditions such as hypertension and diabetes improved opportunities for timely intervention and reduced long term complications.
